When registering make sure to bring:


  • A Florida picture id;
  • Copy of your arrest report (readable copy which you can obtain from the clerk's office or your attorney);
  • Copy of your DUI ticket;
  • Breath/Blood/Alcohol Level reading;
  • Court Order, if they have been convicted.

Level 1 - This is for people who have never been arrested for an alcohol or drug related driving offense in their lifetime.


Level 2 - This is for people who have had more than one alcohol or drug related driving arrest in their lifetime in any state.
